Description
Substance use is a complex, chronic condition that increases the risk of morbidity and mortality for patients of all ages, yet it typically remains under-treated or diagnosed in primary care settings. Providers can adopt innovative, transformative solutions, from episodic interventions to collaborative care models (CoCM), to integrate virtual specialty care into the primary care setting. This presentation will present compelling reasons why high-quality, virtual specialty care can elevate the role of primary care providers in their responses to patients living with substance use disorders. This discussion will uncover why substance use disorders are often assessed and addressed in a disjoined manner, and why the rise in poly-substance use issues will only increase primary care provider challenges. We will then explore how virtual technology – from episodic interventions to full-integrated CoCM – helps advance the role of primary care providers and empower care teams. Finally, we will explore specific use cases and workflow applications that those in the audience should consider when expanding behavorial health care for their patient populations.

Ludwing Salamanca, MD, PhD is an assistant professor of clinical psychiatry at the Valgelos College of Physicians and Surgeons at Columbia University. He completed his psychiatry residency at Mount Sinai Morningside – West in New York City, a fellowship in public psychiatry at the New York State Psychiatric Institute, holds a PhD in public health, and has conducted research on the epidemiology of substance use disorders in the United States. Dr. Salamanca has presented his research and lectured on behavioral topics both nationally and internationally. He is the Collaborative Care Medical Director for RubiconMD and leads quarterly behavioral healthcare CME webinars.

Objectives
- By the end of this presentation, participants will be able to identify the reasons behind the disjointed assessment of substance use disorders in primary care, and understand why the escalating prevalence of polysubstance use issues will intensify challenges.
- Participants will also be able to describe three emerging tech-enabled, virtual specialty models that can be applied in primary care to help care teams build capacity for addressing substance use disorders.
- Finally, participants will evaluate use cases and practical workflow applications for expanding behavioral health care access, and be able to demonstrate a comprehensive understanding of the topic.
Outline
Today’s challenges: epidemiology & relevance in primary care- In the first part of the discussion, Dr. Salamanca will set the stage for the audience, establishing why improving access to SUD treatment in primary care is so critical in improving long term patient outcomes. Dr. Salamanca will summarize how SUD are typically addressed by PCPs today, highlighting the gaps in care, along with opportunities for closing gaps through virtual technology interventions that enhance the role of the primary care provider.
- Dr. Salamanca will explore the importance of a multidisciplinary team approach and of care management in helping to navigate resources (including support through consultant psychiatrists, and the application of the SUD Treatment Algorithms), and why increased access and improved communications can be supported through virtual interventions.
- Dr. Salamanca will explore best practices for building SUD capacity in primary care settings. Examples of assessment types and techniques will be shared, along with strategies for partnering with healthcare administrators to develop more effective ways of implementing virtual specialty care models. Dr. Salamanca will also share recommendations for evaluating and determining the Level of Care for patients with SUD, including concomitant medical issues, and social and environmental factors.
- After setting the stage for the importance of effectively addressing SUD in primary care, along with best practices for building overall capacity, Dr. Salamanca will explore three different virtual specialty care service delivery interventions: psychiatric eConsults, proactive psychiatric eConsults with policy-based workflows, and Collaborative Care. For each strategy type, Dr. Salamanca will describe the benefits, unique considerations, and limitations, as well as program workflow applications, and practical use case recommendations. By the end of the session, participants will have a better understanding of how virtual specialty care technology can safely and effectively be implemented to improve patient access and outcomes, while optimizing in-person care by provides. When possible, pilot data results will be presented to help emphasize the value of each strategy.
